You can send the intake form from within eDoula.biz or you can send your client a link by text message, messaging app or email. When your client submit the intake form, it will populate the fields in their profile and you will receive an email notification.
Sending the intake form from eDoula.biz
Select Clients on the left-hand toolbar.
From the client list, click the client name to open their profile.
In the bottom right corner, click the lightning bolt icon.
Select SEND FORM.
Click the down arrow under Form to select which form to send.
Editing the Message field is optional.
Hit SEND.
Sending the intake form by text message, messaging app or email
Select Clients on the left-hand toolbar.
From the client list, click the client name to open their profile.
In the bottom right corner, click the lightning bolt icon.
Select SEND FORM.
Click the down arrow under Form to select which form to send.
After you choose the form, click COPY LINK.
Close down the pop-up window, do not save.
Paste the link into the message you are sending to your client.
Please note, this link is unique for this client and intake form. This link cannot be sent to other clients.
